Community Development

The Division of Geriatrics in conjunction with the SERGP fosters individual, agency and community partnerships to enhance the competency of local and individuals communities to respond to the complex health needs of older people. A number of key principles have evolved  to facilitate knowledge and information exchange including:

  • Involving clinical leaders, administration and front line workers in assessment,
    identification and planning for response to the clinical and education needs

  • Recognizing and building upon existing strengths and leaders

  • Longitudinal involvement and support

  • Collaborative partnerships with others (Geriatric Psychiatry, CCACs, DHC’s etc)

  • Seeking opportunities to combine focused clinical consultations and educational activities including structured use of site visits and distance consultation/education strategies

  • Building on successes and learning from mistakes

A number of educational workshops have been offered to regulated and non-regulated health care providers, communities and the general public to influence knowledge, practices and attitudes in a manner that will improve health care outcomes for older people.
