Who We Are

The Acute Care Consult Team, based at Kingston General Hospital, is a multidisciplinary team that includes a RN, Geriatrician, OT, PT, SW and Clinical Nurse Specialist (CNS). 

What Do We Offer

At the request of physicians from Kingston General Hospital (KGH) and Hotel Dieu Hospital (HDH), members of the Acute Care Consult Team (ACCT) assess hospitalized seniors with multiple health problems to make best practice recommendations to enhance their care while in hospital and following discharge.

Following primary assessment by the RN and Geriatrician, the patient is discussed at the ACCT’s weekly multidisciplinary conference (MDC) at which time involvement of other team members is decided.  Some clients accepted for admission to the Geriatric Inpatient Service at St. Mary’s of the Lake Hospital site, will receive rehabilitative treatment sessions by the ACCT PT and OT while awaiting transfer.

As well as consultation related to assessment and care planning and management, the ACCT works directly with clients/families and other RGP services to promote optimum transition from acute care to other RGP and community services.  If appropriate, the team will visit patients at home after discharge from hospital. 

In addition, the ACCT works in collaboration with acute care staff to influence care of seniors through educational initiatives, committee and project work, and research and evaluation.

Eligibility

The ACCT assesses seniors with complex multiple health programs that are hospitalized at Kingston General Hospital and Hotel Dieu Hospital.

How to Refer

Referrals should be directed to the RGP Intake Secretary at 613-544-7797 or 1-800-214-5848 or by FAX at 613-544-4017. Physicians should complete a Physician Referral Form (see PDF attached) and fax to the Intake Secretary at 613-544-4017.

Post-Referral Process

A letter is often sent to the family physician to inform him/her:

  • That RGP’s ACCT has been involved in the patient’s care

  • That, although referred, patient was not assessed due to discharge

  • Of referral to other RGP services or to offer recommendations about care management following discharge from acute care
