Who We Are

This 14-space outpatient service is provided by team members that include the Care Coordinator, RN, Pharmacist, Physician, PT, OT and OT Assistant, SW, Therapeutic Recreationist, Secretary, Adjuvant Aide and Manager. The team collaborates closely with the client’s family physician and provides referral and recommendations upon completion of the individual treatment programs.

What Do We Offer

The Day Hospital service provides multidisciplinary geriatric assessment and rehabilitation for frail seniors with complex health issues. Clients initially complete three morning assessment visits and following a team conference, the client receives recommendations, including potential invitation to participate in twice-weekly afternoon treatment programs. Care planning and length of stay is client centered and individualized.

This service also includes group programming focusing upon fall prevention and mobility for safety, Wellness and Safety as well as psycho-educational programs for seniors and their informal caregivers.

Eligibility

Clients in the Southeastern Ontario region are usually referred from other specialized geriatric services when initial assessment has identified rehabilitation opportunities will optimize client quality of life, increase independence or improve functional wellbeing and safety. Clients will also potentially benefit from a multidisciplinary approach.

How to Refer

Referrals for potential clinic appointments should be directed to the RGP Intake Secretary at 613-544-7797 or 1-800-214-5848 or by FAX at 613-544-4017. Physicians should complete a Physician Referral Form (see PDF attached) and fax to the Intake Secretary at 613-544-4017.

Post Referral Process

There is ongoing communication with the family physician during the client’s participation in Day Hospital, as well as a discharge summary. A typical timeline is for clinic letter to be sent within 10 working days.
