Geriatric Inpatient Service (Geriatric Rehabilitation)

Who We Are

The inpatient team for the 16-bed inpatient unit at PCCC’s St. Mary’s of the Lake Hospital site is composed of members from Dietetics, Discharge Planning, Medicine, Nursing, Occupational Therapy, Pastoral Care, Pharmacy, Physiotherapy, Psychology, Social Work, Recreation Therapy and other therapies.

What Do We Offer

The inpatient service provides short term, comprehensive hospital assessment, treatment and rehabilitation for clients referred from other  component of  Specialized Geriatric Services. The goal of inpatient rehabilitation is to optimize the client’s health, functional independence and quality of life. Client and family are active participants in the rehabilitative process and the majority of clients are discharged back to their pre-admission place of residence.

Eligibility

Patients in Southeastern Ontario with complex health needs that are likely to be improved or reversed as a result of inpatient assessment and treatment are eligible for this service.

How to Refer

Referrals should be directed to the RGP Intake Secretary at 613-544-7797 or 1-800-214-5848 or by FAX at 613-544-4017. Physicians should complete Physician Referral Form (see PDF attached) and fax to the Intake Secretary at 613-544-4017. An assessment will be usually be carried out within Outreach or consult team to determine suitability.

Post Referral Process

A discharge summary, including ongoing care recommendations, is forwarded to the family physician and other service providers. A typical timeline is for the letter to be sent within 10 working days.
